How to I get an extension on a Restraining Order After Hearing to Stop Harassment?

I have a Restraining Order After Hearing to Sop Harassment that is going to expire next Friday. The Order has been violated twice, once 30 days after it was issued and again last week. Both violations were validated with police reports. The first violation was forwarded to the DA but nothing became of it. When the order was placed a year ago, the judge told me to contact the court if the order was violated and it would be extended. I can't seem to locate a form/document to extend the order. When I went to the court house this week, I was told my only option was to reapply for a new order. Does this sound correct? Thank you for your help.
